Mandatory Internship in Rural Areas
indiandoctor4u//New delhi Bureau// At present, the Government is not considering any proposal for mandatory internship for medical professionals in rural and remote areas. Health being a State subject, provision of healthcare facilities falls under the jurisdiction of respective State Governments. However, to encourage doctors working in remote and difficult areas, the MCI with the previous approval of Central Government, has amended the Post Graduate Medical Education Regulations, 2000 to provide :- (i) 50% reservation in Post Graduate Diploma Courses for Medical Officers in the Government service, who have served for at least three years in remote and difficult areas; and (ii) Incentive at the rate of 10% the marks obtained for each year in service in remote or difficult areas as upto the maximum of 30% of the marks obtained in the entrance test for admissions in Post Graduate Medical Courses. Further, under, NHM, financial incentive is also provided to MBBS as well as PG doctors for serving in the rural areas. These incentives are over and above the salaries of the doctors concerned. The Minister of State (Health and Family Welfare), Sh Faggan Singh Kulaste stated this in a written reply in the Rajya Sabha.

Rabies Vaccination Under National Rabies Control programme(NRCP)

Funds under NRCP are only provided for training, surveillance, laboratory strengthening and advocacy etc. However for rabies vaccination all the states/UTs have been communicated to include Anti- Rabies vaccine (ARV) and Anti Rabies serum ( ARS ) under essential drug list and to undertake the procurement of ARV and ARS under National Free Drug service initiative under National Health Mission. As there is rabies elimination drive by 2030 globally, there is significant demand of Anti Rabies Vaccine and Serum from the neighboring courtiers like Myanmar, Malaysia and Philippines and these countries are importing vaccine from India.Many states and stakeholders have pointed out that preferential export of ARV by Indian Manufactures at escalated prices to the neighboring countries has resulted in nonresponse and non-participation by the manufactures in domestic tenders. Drug Controller General of India (DCGI) was directed to monitor the pharmaceutical firms producing ARV for ensuring regular supply of ARV to the States. DCGI requested all the manufacturers to furnish monthly details of the quantities of ARV manufactured, exported, sold in the domestic market and supplied to the government institutions. Answer –As per the latest information available by the government sources the most accepted and commonly used method of diagnostic for SARS-CoV2 Variant is RT-PCR method. With the help of this test currently several centres are detecting the virus. This method detects specific genes in the virus, such as Spike (S), Enveloped (E) and Nucleocapsid (N) etc to confirm the presence of virus. However, in case of Omicron, as the S gene is heavily mutated, some of the primers may lead to results indicating absence of the S gene (called as S gene drop out). This particular S gene drop out along with the detection of other viral genes could be used a diagnostic feature of Omicron. However, for final confirmation of the omicron variant genomic sequencing is required.

UK now approved a variant-adapted coronavirus vaccine

The UK medicines regulator gave the conditional approval for its use as a booster for adults. This was decided after clinical trial data showed the booster triggered a strong immune response against both Omicron (BA.1) and the original 2020 virus. According to Company Moderna in June 2022, the sources said trial data showed that when given as a fourth dose, the variant-adapted shot raised virus-neutralising antibodies by eight-fold against Omicron. Moderna trial data showed its variant-adapted booster generated virus-neutralising antibody levels against the subvariants that were 1.69 times higher than those given the original booster.
